This may have a better spot in the forum to be, I think I had seen one a while back but I cannot find a spot for the hints and tips... or could go with leaky water.
I have learned another lesson with leaky maps after 1400 hours on hammer lol. I recently made a map and it was an underground map with no skybox in it and had water lying in the low areas. I made a Trigger_waterydeath and I compiled the map and it all the sudden had a leak. I loaded the pointfile but it didn't have one to load so I clicked on the entity and the little circle that signifies the center of the entity was out in the black area outside of my level so I moved it in... it works now... I also rotated a whole level at one time and the buttons "circle" didn't move with it and made a leak so I moved them back to the button...
Lesson learned

Entity Origins, or more generally Helper Nodes, being outside of the map is a common cause of leaks.
